Registering for the Patient Portal

When entering the patient portal for the first time, you must register and activate your account. You can register for the portal in one of two ways:

  1. Self-registration

  2. Register with a patient portal Invite

Self-registration

To complete self-registration, follow these steps:

  1. Click on the Patient portal link and you will be directed to the portal

  1. Click Register to begin the self-registration process for the portal
  1. After filling out the required information on the screen and clicking Register, you will receive a Patient Portal Account Notification email with a link to the Complete Account Registration screen.

Please follow the instructions below in the Register with a Patient Portal Invite section to complete the patient portal account registration.  

Register with a Patient Portal Invite

When you click Invite in the patient portal dialog, an email is sent to the email address in the Account Email field. The email contains a link that takes you directly to the Complete Account Registration screen.

The following information must be entered to register and activate a portal account:

  • New Password

Note: Passwords must contain a minimum of eight characters and include one upper case letter, one lowercase letter, one number, and one special character.

  •  The following identifier:
    • Birth Date – MM/DD/YYYY

 

  1. Read and agree to the Terms & Conditions then Click Register .
  1.  Once the account is registered and activated, you are automatically logged into the patient portal.

 

The patient portal is the place for you to fill out all forms that are needed, send us messages, check upcoming appointments and pay any remining balances.

Please note that it is important to go to the portal prior to each and every appointment. We ask that you fill out your pre-visit questionnaire to report your current symptoms, treatments, and questions for the doctor.
